Application forms
All the forms available on this site are in either of two formats.
Adobe Acrobat PDF format or Microsoft RTF format
PDF Allows you to print the application forms from any machine on any platform - Mac or PC but requires Adobe Acrobat to be on your computer.
RTF allows you to edit and pre-fill the application forms from many machines on many platforms. Refer to the FAQ on printing and forms for technical help.
Please download the appropriate form from the options below. Please print out single sided. If you are unclear which form you should complete please click on the criteria links first.
All forms must be accompanied by the non refundable fee and other documentation identified in the completion guide.

Use a TC1 if you are newly graduated from an approved teachers education programme, or
your last practising certificate was issued "Subject to Confirmation" STC; or
your last practising certificate was issued Provisional; or
you are an experienced New Zealand teacher who does not currently meet the criteria for full registration; or
you are an experienced New Zealand teacher who has worked overseas for more than three years in the last five years; or
you are an overseas teacher seeking registration and a practising certificate for the first time in New Zealand. Fee $120.00.

Use a TC2 if you already hold provisional or subject to confirmation registration and you have completed the requirements to move to Full Registration. You must hold a current practising certificate to be eligible to move to Full Registration as this application is for a change of registration category only. It does not affect the expiry date of your practising certificate. Fee $80.00

Use a TC3 if you have completed two years teaching service in the past five years in the general education system of New Zealand or in an approved setting. Fee $120.00

Use a TC4, if you are not eligible for registration as a teacher in New Zealand but have been offered a position to teach a specific subject in a specific school for a specific time.
- $40 for up to a 1 year LAT
- *$80 for up to a 2 year LAT
- *$120 for up to a 3 year LAT
