Template letter for making a mandatory report about possible serious misconduct by a teacher

This template letter is for reporting to the NZ Teachers Council if you have reason to believe that a registered teacher has engaged in serious misconduct. It can be combined with another mandatory report letter if necessary. You can copy and paste it into your own wordprocessing programme to edit and print out on letterhead. You should send this report to the NZ Teachers Council as soon as possible after determining that you have reason to believe the teacher has engaged in serious misconduct.

  • "Relevant documentation" means documentation that provides evidence of the matters complained of or reported about, including but not limited to:

(a) Statements of complainants, witnesses and any other persons;
(b) Letters of complaint received by the employer about the teacher;
(c) Recent performance appraisals of the teacher;
(d) Reports of independent investigators;
(e) Letters between the employer or professional leader and the teacher concerning the complaint or matter of concern;
(f) Any statement or responses, or records of these statements or responses received from the teacher or his or her advocate;
(g) The teacher's letter of resignation (if applicable);
(h) Minutes of Board meetings at which the matter was discussed;
(i) Any information provided by other persons or by the police.
