Form for making a complaint about a teacher

This form is for making a complaint about the conduct, competence, or mental or physical fitness of a registered teacher or holder of a limited authority to teach. Please complete all questions in this form.

If you want to complain about more than one registered teacher, please complete one complaint form for each registered teacher about whom you wish to complain. Copies of this form and any documents you supply to the NZ Teachers Council will be provided to the teacher you are complaining about.

You should make your complaint to the principal or Board of Trustees of the teacher's school before contacting the Council. The Council can only consider your complaint if the Board was unable to deal with the complaint satisfactorily.

  documents to the NZ Teachers Council (PO Box 5326 Wellington,
  marked for the attention of The Legal Adviser). If you
  cannot provide the documents, please provide details of who has
  access to the documents, their contact details and what
  information they may be able to provide:
Investigation by the employer (*): If you have referred
  this complaint to the teacher's employer (eg the Board of Trustees),
  what was the outcome of the complaint? If you have not referred
  this complaint to the teacher's employer, what were your reasons
  for not doing so?
